Reside in an eligible province (ON)
Be at least 65 years old
Net family income under $42,290
Not have equivalent private insurance
How to apply
File the previous year's income tax return so income can be verified.
Apply online or by mail; spouses or common-law partners must submit two separate applications.
Renew coverage every year because it ends on July 31.
