How to apply
Check the dependency relationship: spouse, child, grandchild, or parent, grandparent, sibling, uncle, aunt, nephew or niece who lived in Canada at any time in the year.
Get a signed statement from a medical practitioner giving the onset and expected duration of the impairment, unless an approved T2201 is already on file.
Claim the amount on the matching line of the federal return, according to the dependant's age and your relationship.
